*Obtaining Your Card: The USA PATRIOT Act is a federal law that requires all financial institutions to obtain, verify and record information that identifies each person who opens a Card Account. What this means for you: When you open a Card Account, we will ask for your name, address, date of birth and other information that will allow us to reasonably identify you. We may also ask to see your driver's license or other identifying documents at any time. “Guaranteed Approval” is contingent upon successfully passing this mandatory identification confirmation.

These Cards are issued by The Bancorp Bank pursuant to a license from Visa U.S.A. Inc. and may be used wherever Visa debit cards are accepted. The Bancorp Bank; Member FDIC.

¹ The Monthly Fee for this Card is $11.90. If you received a direct deposit in that given month, you will only receive a $6.95 Monthly Fee. If you do not receive a direct deposit in that given month, you will be assessed the additional $4.95 as a Conditional Monthly Fee.

² An additional Load Fee (depending upon retailer) will be charged by the retailer each time funds are loaded on your Card. This amount is an independent fee assessed by the individual retailer only and is not assessed by us.

³ You may withdraw cash from a bank teller at any one of 14,000 Visa member banks within the United States. No additional fees apply. You must present a valid form of identification along with your Card to the bank teller and let him/her know the exact amount you wish to withdraw. The maximum amount you can withdraw is the available balance on your Card up to $2,500.00. If you attempt to withdraw an amount greater than your available balance, the transaction will decline. Please note the bank teller cannot access your account to check your available balance.

⁴ Using this feature to send money from your Card Account to a recipient without a Vision Prepaid Visa Card is currently unavailable. You may, however, continue to use this feature to send money from your Card Account to a recipient with a Vision Prepaid Visa Card.

⁵ If certain conditions are not met, your Card will be considered inactive. Inactivity is determined by the fee plan you chose. (a) STANDARD: Inactivity is defined as no value loads, no purchase activity or cash withdrawals performed for one hundred eighty (180) consecutive days.
(b) DIRECT DEPOSIT: Inactivity is defined as no direct deposit being performed on your Card Account in the previous month.
(c) LIFETIME VALUE: Inactivity is defined as less than $1,000.00 in total purchase transactions in the previous month.If your Card becomes inactive, your Card will be charged a Conditional Monthly Fee each month with the first occurrence assessed after the first full month of inactivity. Thereafter the Conditional Monthly Fee will be assessed as activity restarts or a zero (0) balance is reached. The Conditional Monthly Fee will be assessed on the first day of each month for the STANDARD pricing plan and the third calendar day of each month for the DIRECT DEPOSIT and LIFETIME VALUE pricing plans.

⁶ A promotional fee may apply. A promotional Activation Fee is $51.65 for cards that have not been loaded. A Promotional Fee is $41.70 for previously loaded cards. If a promotional fee is charged, the Monthly Fee is reset to $0.

All fee amounts will be withdrawn from your Card Account and will be assessed as long as there is a remaining balance on your Card Account, except where prohibited by law. Anytime your remaining Card Account balance is less than the fee amount being assessed, the balance of your Card Account will be applied to the fee amount.
